Welcome to Louisburg College Athletic Training! We are so excited for you to join your team! There is a couple of steps you need to take before you will be permitted to participate in your sport.
First-Years
1) Make a sportswear account for step-by-step instructions click HERE
2) Get a physical
a. You can access the physical form HERE. We cannot accept high school physicals.
b. There is a HISTORY section and a PHYSICAL EXAM section of the physical paperwork. All parts of the physical MUST be
completed. (Fill out the history paperwork BEFORE your appointment for review by the physician.)
c. The physical must be completed by a qualified medical professional (MD,NP,LPN,ect.)
d. Bring the completed physical paperwork to the Athletic Trainer.
e. Physicals expire every 13 months.
f. Athletes will not be cleared to participate until the Athletic Trainer has your physical.
3) Concussion Testing
a. Louisburg College Athletic Training uses an online platform called IMPACT for baseline concussion testing in coordination with our supervising physicians and concussion specialist at Ortho NC.
b. Prior to the start of your athletic participation at Louisburg College you are required to complete a concussion baseline test.
c. A link will be sent from IMPACT to the email that you provided your coaches (make sure you check your spam and junk mail). If you do not see it then you may come visit the athletic trainers/reach out to us and we will provide another link.
d. This test takes 20-30, must be done on a computer, and requires you to take your time.
e. The athletic trainers will be informed by IMPACT if you passed or failed. You will be notified if you failed and will be required to take it again after 24 hours of your first test.
f. You will not be cleared to participate in any athletic activity until you have passed the concussion baseline test.
Second-Years
1) Get a new physical (they expire every 13 months and you will not be cleared to participate until we have an updated physical on file.)
2) Make sure your SportsWare information is updated.
